Changes to the checkout of Jan van Haasten consisting of the following:

1. Split between step billing address and shipping address
2. Let customers specify a scheduled shipping date in the shipping method step
3. Integration with dutch postal code lookup form Pro6PP during checkout

This extension is developed specifically for Jan van Haasteren. Feel free to re-use
